STANDARD MEMBERSHIP AGREEMENT TERMS
Membership Terms, Billing & Recurring Payment Authorization | By purchasing this membership, I authorize NEXUS Martial Arts to charge my selected payment method for the initial purchase and all recurring membership payments until it is canceled IN WRITING, BY EMAIL ONLY. Billing will continue unless my membership is canceled by email to info@nexus-az.com in accordance with this Agreement. No other form of cancellation will be accepted. Missed classes, dissatisfaction with services, schedule conflicts, relocation, illness, injury, revoking or replacing my payment method, disputing charges with my bank and/or personal circumstances do not cancel this Agreement or qualify for refunds, charge reversals, or release me from my payment obligations under this Agreement.
By checking this box, I confirm the terms above were presented to me prior to purchase. I understand this is a recurring membership that will continue to bill monthly until properly canceled & authorize monthly charges until cancellation is effective. Cancellation MUST be submitted by email to info@nexus-az.com and requires 30 days’ notice. Verbal/in-person requests, phone calls, texts/social media messages and/or stopping attendance do not cancel my membership or payment obligation.*
