The information below outlines key policies, procedures, expectations, membership terms and liability conditions that apply to any individual entering and/or participating in any NEXUS Martial Arts activity. By visiting NEXUS Martial Arts and/or participating in any class, event, open mat, trial session, or activity, I acknowledge and agree to the following:

Assumption of Risk & Participation Terms | I understand and accept martial arts training involves inherent risks, including physical contact and strenuous activity that may result in serious injury, disability, or death. These risks may arise from my own actions, the actions or negligence of others, program rules, facility conditions, or equipment use. Additional, unforeseen risks may also exist. I agree to inspect all facilities, equipment and training partners before participating. If anything appears unsafe or beyond my ability, I will notify an instructor immediately and refrain from participating until the concern is resolved. I voluntarily assume all risks and accept full responsibility for any injury, disability, or death that may result from participation. I agree to follow all training and safety rules. I certify I’m in good physical condition and not advised by any medical or mental health professional to avoid physical activity. I authorize qualified medical personnel to administer emergency care should it become necessary. I release, waive and discharge NEXUS Martial Arts; its owners, instructors, staff, agents, volunteers, affiliates, members and the owners or lessors of the premises from all claims, demands, losses, or damages arising from injury (including death) or property damage, whether caused by negligence or otherwise, on behalf of myself, my heirs and next of kin.

Health & Medical Acknowledgment | You confirm you are physically and mentally able to participate and that no medical or mental health professional has advised you not to train. If an emergency occurs, you authorize NEXUS to seek medical treatment on your behalf and release NEXUS from any liability related to that care.

Release & Waiver of Liability | You agree to release and hold harmless NEXUS, its owners, instructors, coaches, staff, volunteers, affiliates and property owners from any claims related to injury (including death) or property damage, whether caused by negligence or otherwise. This release applies to you, your heirs and legal representatives.

Membership Terms, Billing & Authorization | You authorize NEXUS to charge your payment method according to your selected purchase. Billing will continue unless your purchase is properly CANCELED BY EMAIL. No other form of cancellation will be accepted. Missed classes, dissatisfaction with services, schedule conflicts, relocation, illness, injury and/or personal circumstances do not qualify for refunds or charge reversals.

Membership Pause Policy | This membership may be paused for a $45/month fee. Pause requests must be submitted in advance and in writing by email to info@nexus-az.com or x@nexus-az.com and are subject to approval. Billing will automatically resume at the end of the approved pause period unless otherwise confirmed in writing. Pause fees are non-refundable.

Cancellation Policy | If you cancel this membership before completing the full term, you will be responsible for an early termination fee equal to 50% of the remaining contract balance, calculated at the time your cancellation email is received. This fee is due in full at cancellation. To cancel your membership written notice must be sent by email to info@nexus-az.com or x@nexus-az.com at least 30 days prior to your next billing date. EMAIL ONLY. We do not accept cancellation requests made in person, verbally, by phone, text, or through social media. If proper notice is not received within the required timeframe, the next billing cycle will be charged the full amount and considered valid and non-refundable.

Refunds & Chargebacks | You agree not to file a chargeback for charges that comply with these Terms & Conditions. In the event of a dispute, this agreement, payment records and communication history may be used as evidence. Improper chargebacks may result in additional fees where permitted by law.

Photo, Video & Media Consent | You grant NEXUS permission to photograph and/or record you and/or your child(ren) during classes or events and to use that content for marketing, promotional, or educational purposes. NEXUS retains ownership of all media. No compensation will be provided and you waive any right to approve final materials.

Final Acknowledgment | You confirm you have read and understand these Terms & Conditions. By subscribing to this 12-month membership, you accept the terms and acknowledge they are fair, binding and enforceable.
