YOUTH MEMBERSHIP AGREEMENT TERMS
By enrolling a minor in a youth membership, registering the minor for any class, event, private lesson, open mat, or activity and/or allowing the minor to use the facilities at NEXUS Martial Arts, I acknowledge I have read, understand and agree to the following Terms & Conditions on behalf of the minor participant:
By checking this box, I confirm these terms were presented to me before purchasing this membership on behalf of the minor. I understand this is a recurring membership & authorize recurring charges until the membership is properly canceled. I understand cancellation must be submitted by email to: info@nexus-az.com, a 30-day cancellation notice is required & verbal/in-person requests, phone calls, text/social media messages and/or stopping the minor's attendance don't cancel the membership.*
By signing, I acknowledge I have read, understand and agree to the terms, conditions, policies and procedures associated with this waiver and all NEXUS Martial Arts purchases.
