YOUTH MEMBERSHIP AGREEMENT TERMS
Membership Terms, Billing & Recurring Payment Authorization | I authorize NEXUS Martial Arts to charge my selected payment method for the initial purchase and all recurring membership payments associated with the minor's membership until it is properly canceled. Billing will continue unless the membership is canceled in writing by email to info@nexus-az.com in accordance with this Agreement. No other form of cancellation will be accepted. Missed classes, dissatisfaction with services, schedule conflicts, relocation, illness, injury, revoking or replacing my payment method, disputing charges with my bank and/or personal circumstances do not cancel this Agreement or qualify for refunds, charge reversals, or release me from my payment obligations.
By checking this box, I confirm these terms were presented to me before purchasing this membership on behalf of the minor. I understand this is a recurring membership & authorize recurring charges until the membership is properly canceled. I understand cancellation must be submitted by email to: info@nexus-az.com, a 30-day cancellation notice is required & verbal/in-person requests, phone calls, text/social media messages and/or stopping the minor's attendance don't cancel the membership.*
