By enrolling a minor in a youth membership; registering for any class, event, private lesson, open mat, or activity; and/or allowing the minor to use the facilities at NEXUS, the parent or legal guardian confirms they have read, understand and agree to the terms & conditions below, as well as any policies and/or procedures specific to the selected purchase(s), on behalf of the minor participant:
Assumption of Risk & Participation Terms | Martial arts training involves inherent risks, including but not limited to physical contact, strenuous activity, training equipment and interaction with other participants. These risks may result in serious injury, permanent disability, death, or other loss. The parent or legal guardian understands that these risks may arise from the actions of the minor, the actions of others, facility conditions, equipment use, or other unforeseen factors. By allowing the minor to participate, the parent or legal guardian voluntarily assumes all risks on behalf of the minor and accepts full responsibility for any injury or loss that may occur.
Health & Medical Acknowledgment | The parent or legal guardian confirms that the minor is physically and mentally capable of participating and that no medical or mental health professional has advised against participation. In the event of an emergency, the parent or legal guardian authorizes NEXUS to seek medical treatment for the minor and releases NEXUS from any liability related to such care.
Release & Waiver of Liability | The parent or legal guardian agrees to release, waive and hold harmless NEXUS, its owners, instructors, coaches, staff, volunteers, affiliates and property owners from any and all claims related to injury (including death) or property damage involving the minor, whether caused by negligence or otherwise. This release applies to the minor, the parent or legal guardian and their heirs and legal representatives.
Membership Terms, Billing & Authorization | The parent or legal guardian authorizes NEXUS to charge the provided payment method according to the selected youth membership or purchase. Billing will continue unless properly CANCELED BY EMAIL. No other form of cancellation will be accepted. Missed classes, dissatisfaction with services, schedule conflicts, relocation, illness, injury, or personal circumstances do not qualify for refunds or charge reversals.
Membership Pause Policy | This membership may be paused for a $30/month fee. Pause requests must be submitted in advance and in writing by email to info@nexus-az.com or x@nexus-az.com and are subject to approval. Billing will automatically resume at the end of the approved pause period unless otherwise confirmed in writing. Pause fees are non-refundable.
Cancellation Policy | If you cancel this membership before completing the full term, you will be responsible for an early termination fee equal to 50% of the remaining contract balance, calculated at the time your cancellation email is received. This fee is due in full at cancellation. To cancel your membership written notice must be sent by email to info@nexus-az.com or x@nexus-az.com at least 30 days prior to your next billing date. EMAIL ONLY. We do not accept cancellation requests made in person, verbally, by phone, text, or through social media. If proper notice is not received within the required timeframe, the next billing cycle will be charged the full amount and considered valid and non-refundable.
Refunds & Chargebacks | The parent or legal guardian agrees not to file a chargeback for charges that comply with these Terms & Conditions. In the event of a dispute, this agreement, payment records, and communication history may be used as evidence. Improper chargebacks may result in additional fees where permitted by law.
