1. AGREEMENT TO TERMS

The information below outlines the key policies, expectations, visitor access terms and liability conditions that apply to any individual entering or participating in activities at NEXUS. By visiting NEXUS and/or participating in any class, event, open mat, trial session, or activity, the visitor acknowledges and agrees to the following:

2. ASSUMPTION OF RISK

I understand and accept martial arts training involves inherent risks, including physical contact and strenuous activity that may result in serious injury, disability, or death. These risks may arise from my own actions, the actions or negligence of others, program rules, facility conditions, or equipment use. Additional, unforeseen risks may also exist. I agree to inspect all facilities, equipment and training partners before participating. If anything appears unsafe or beyond my ability, I will notify an instructor immediately and refrain from participating until the concern is resolved. I voluntarily assume all risks and accept full responsibility for any injury, disability, or death that may result from participation. I agree to follow all training and safety rules. I certify I’m in good physical condition and not advised by any medical or mental health professional to avoid physical activity. I authorize qualified medical personnel to administer emergency care should it become necessary. I release, waive and discharge NEXUS; its owners, instructors, staff, agents, volunteers, affiliates, members and the owners or lessors of the premises from all claims, demands, losses, or damages arising from injury (including death) or property damage, whether caused by negligence or otherwise, on behalf of myself, my heirs and next of kin.

3. VISITOR ACCESS, MEMBERSHIP & PURCHASE INFORMATION

I understand NEXUS membership options, class offerings and class pack options are available for review on the NEXUS website at www.nexus-az.com. If I choose to purchase or enroll in any membership, class, or class pack at any time, it is my responsibility to read and understand the specific terms associated with that option. Each membership option carries its own cancellation and pause policies. The X Pass, Striking Pack, individual classes and drop-in day passes are non-refundable and non-cancelable and no cancellation terms apply to these purchases. By signing this document, I confirm I understand all membership, class and product options are outlined on the NEXUS website and that, should I choose to enroll or make a purchase at at later time, I agree to all terms, policies and conditions associated with the membership/s or product/s I select.

4. PHOTO | VIDEO | SOCIAL MEDIA CONSENT

I authorize NEXUS and its representatives to photograph and/or film me and/or my child(ren) and use all assets for marketing, promotional, and/or social content. I understand NEXUS owns all photos and footage and may use and/or distribute them without limitation or compensation. I waive all rights to inspect or approve final materials and release NEXUS from any claims related to the use of this media.

5. FINAL ACKNOWLEDGMENT

I’ve read this visitor overview, liability waiver, release of liability and assumption of risk. I fully understand its contents and acknowledge I’m giving up substantial rights by signing. I understand this waiver is binding and my presence and/or participation at NEXUS confirms my agreement to all policies, terms and safety requirements listed above. I’m signing voluntarily, without inducement or guarantee and intend my signature to serve as a complete and unconditional release of all liability, as well as acknowledgment of the details outlined above.
