YOUTH MONTH-TO-MONTH MEMBERSHIP AGREEMENT TERMS

By enrolling a minor in a youth month-to-month membership, registering the minor for any class, event, private lesson, open mat, or activity and/or allowing the minor to use the facilities at NEXUS Martial Arts, I acknowledge I have read, understand and agree to the following Terms & Conditions on behalf of the minor participant:

Assumption of Risk & Participation Terms | I understand martial arts training involves inherent risks, including physical contact, strenuous activity, the use of training equipment, and interaction with other participants. These risks may result in serious injury, permanent disability, or death. I understand these risks may arise from the actions of the minor, the actions or negligence of others, facility conditions, equipment use, or other unforeseen circumstances. By allowing the minor to participate, I voluntarily assume all risks on behalf of the minor and accept full responsibility for any injury or loss that may occur.

By checking this box, I confirm these terms were presented to me before purchasing this membership on behalf of the minor. I understand this is a recurring membership & authorize recurring charges until the membership is properly canceled. I understand cancellation must be submitted by email to: info@nexus-az.com, a 30-day cancellation notice is required & verbal/in-person requests, phone calls, text/social media messages and/or stopping the minor's attendance don't cancel the membership.
